Upload Student & Parent Forms
Upload completed ARCTIC forms here. Files should be uploaded as PDF, Word documents, or image files. Contact ARCTIC staff if you need help submitting forms.
How to Upload Forms
- Download and print the required form from the Forms & Documents page — or sign the Consent to Treat, Behavioral Agreement, and Alcohol & Tobacco forms online there.
- Complete the form in full — forms missing signatures or required fields cannot be accepted.
- Scan or take a clear photo of the completed form, or save it as a PDF or Word document.
- Fill in the student and parent/guardian information below, select the form type, and attach your file.
- Submit a separate upload for each form. You will see a confirmation once the form is received.
Good to know
The Consent to Treat, Behavioral Agreement, and Alcohol, Tobacco & Drugs forms can now be filled out and signed online — no printing or uploading needed. Visit the Forms & Documents page to sign them electronically.
